Website and Mobile App Privacy Policy

 

 

 

Midwest Melanoma Partnership (“MMP”) respects the importance of the privacy of your personal information.  MMP provides this Privacy Policy so you know about the online information practices of MMP and understand how MMP treats the information we receive about you from your use of the midwestmelanoma.org website (the “Site”) and MMP’s mobile software applications (“Apps”).

 

 

 

When you use or access the Site or Apps, you agree to the terms of this Privacy Policy.  Your visit to the Site and any dispute over privacy is subject to the Terms of Use [make a hyperlink], including limitations on damages and application of the law of the State of Minnesota.  Your use of the Apps and any dispute over privacy is subject to the applicable End User License Agreement including limitations on damages and application of the law of the State of Minnesota.

 

 

 

Use of the Site and Apps is strictly voluntary.  You may not use the Site or Apps if you do not agree to the Privacy Policy.

 

 

 
  1. The Information MMP Collects

     

    Anonymous Electronic Information.  In general, you can browse the Site without telling MMP who you are or revealing any personal information about yourself.  MMP, however, collects your computer’s or mobile device’s IP address, which you necessarily disclose when you visit the Site or download or use an App so that data (such as the web pages you request) can be sent to your computer or mobile device.  The Site or Apps may utilize "cookies," which are anonymous, unique identifiers that web pages on the Site may use to store data on your computer or mobile device or send data to our web server.  MMP may use cookie data to "recognize" you when you visit the Site or use the App in the future, to troubleshoot how the Site or Apps operates, and for internal analysis of how people use the Site or Apps.  Should you decide not to accept cookies from the Site or Apps, you will limit the functionality MMP can provide you. 

     

    Participating in Activities.  When you engage in certain activities on the Site or App, such as registering as a user of the Apps or sending MMP a message using the Site, MMP may ask you to provide personal and other information.  In order to engage in such activities you will be required to provide information that lets MMP know the specifics of who you are or where you are, and may include, without limitation, your name, email address, age, gender, zip code, ethnicity, and other personal information and if you are a healthcare provider, your NPI number.  If you choose to share any personal information with MMP, you will see that the amount and type of personal information MMP requests depends on how you use the Site or Apps.  Depending upon the activity, some of the information MMP asks you to provide is identified as mandatory and some is voluntary.  If you do not provide the mandatory data with respect to a particular activity, you will not be able to engage in that activity. 

     

  2. The Way MMP Uses Information Collected on the Site and the Apps

     

    MMP may use your IP address to, among other things: (a) help diagnose problems with the Site or Apps, administer and improve the Site or Apps, and measure the use of the Site or Apps; (b) identify the type of web browsers used, the pages viewed, the web sites from which a visitor found the Site or Apps, and the search terms and search engines used to find the Site or Apps; and (c) gather other web analytics information relating to visitors’ use and navigation of the Site or Apps.  This information does not identify you personally. On occasion MMP may aggregate this data with data from other users to track overall visitor traffic patterns and provide this aggregate information to vendors, business partners, or other parties to demonstrate the amount of interest in the Site and Apps and to help MMP plan for technical infrastructure requirements.

     

    MMP may use your personal information for particular activities, including, without limitation, verifying your identity and health care practice credentials, customer service, marketing and research, providing promotional and other information to you, sending you text messages, push notifications, and notifications through the Apps and/or your mobile device, and to contact you regarding administrative notices.  MMP may share this information with its members, affiliates or business partners in connection with any of the foregoing activities.  Each of these parties’ privacy policies will govern how they use your personal information. While we try to be selective in working with third parties, we are not responsible for their use of your personal information. Where payments are involved, MMP does not collect your credit card information.  Instead, you provide your credit card information directly to MMP’s credit card service provider who will collect information for and process your payment for donations to MMP or other payments.  The service provider may have its own terms and conditions and privacy policy.  Except as described in this Privacy Policy, MMP will not give or sell your personal information to any other company for its use in marketing or solicitation.

     

    MMP may use and share aggregate information with vendors, potential advertisers, business partners, or other third parties.  Aggregate information does not include personal information.

     

    MMP may disclose personal information in any of the following circumstances: (a) in response to a subpoena, search warrant, court order, or other legal process or request, or as may otherwise be required by applicable law or regulation; (b) to protect and defend MMP’s rights or property or those of its affiliates or business partners, or others; or (c) as MMP, in its sole discretion, believes is appropriate under exigent circumstances in response to a perceived threat to the personal safety, property, or rights of any person or organization.

     

    In addition, since MMP’s database is a business asset, in the event MMP is reorganized or becomes part of another organization, you consent to the sale, assignment, or transfer of your personal information to a successor that acquires substantially all of the business assets of MMP, or to an affiliate, as applicable.

     

  3. MMP’s Relationship to Other Sites that can be Accessed Through the Site or Apps

     

    The Site or Apps may contain links to other web sites. Additionally, other web sites may contain links to the Site or Apps. We do not review or monitor the web sites linked to the Site or Apps and are not responsible for the contents or privacy policies related to any other web sites. If you visit one of these other web sites, you may want to review the privacy policy on that web site. Where the Site or Apps contains a link to another web site owned and/or operated by us, such web site use may be subject to different and additional terms of service and privacy policy terms and conditions.

     

  4. Security of Your Personal Information

     

    MMP takes what it believes to be reasonable precautions and has implemented certain safeguards to protect the security of data and information.  However, MMP cannot guarantee against any loss, misuse, unauthorized disclosure, or alteration or destruction of data or personal information. While MMP strives to protect your personal information, MMP cannot guarantee the security of any information you transmit to or from the Site or Apps, by email or otherwise, and you provide such information at your own risk.  You acknowledge that: (a) there are security and privacy limitations in computer systems and on the Internet which are beyond the control of MMP; (b) the security, integrity, and privacy of any and all information and data exchanged between you and MMP through the Site or Apps, including personal information, cannot be guaranteed; and (c) any such information and data may be viewed or tampered with by a third party while such information or data is being used, transmitted, processed, or stored.

     

  5. Changes to Privacy Policy

     

    MMP reserves the right to modify this Privacy Policy, in whole or in part, at any time without notice to you. You expressly waive any right to receive notice of any changes to this Privacy Policy.  MMP encourages you to review this Privacy Policy regularly in your use of the Site or Apps to ensure you are aware of current practices.  Any use of the Site or Apps following the date on which MMP implements changes to this Privacy Policy shall constitute your acceptance of all such changes.
